Nourish Care is a secure digital care planning system used by Care Assist to record, manage and monitor care and support. It helps ensure that care is person‑centred, consistent, and responsive to individual needs.
Through Nourish Care, our staff record:
Daily care and support notes
Health and wellbeing updates
Medication administration records (where applicable)
Outcomes, goals and progress
Risk management and safety information
This system supports high‑quality care and helps us work transparently with families, professionals, and commissioners.
How Your Information Is Used
Information recorded on Nourish Care is:
Used only to plan, deliver and review your care and support
Stored securely in line with UK GDPR and data protection legislation
Accessed only by authorised staff and relevant professionals

Some people we support, family members, or authorised representatives may be given access to view information on Nourish Care.
